The website: www.Elite2ndREAD and/or the company: EliteRad Radiology Services will store your personal and medical information, including but not limited to, radiological images for as long as necessary to render the second opinion report.  After the second opinion report is created, any tangible radiology films may be returned to you provided that you have a) requested the return of films and, b) provided a self-address stamped envelope for mailing.

The website: www.Elite2ndREAD and the company: EliteRad Radiology Services is not records custodian and as such is not responsible for storing, filing, or safe keeping medical, personal or imaging information including imaging, after a second opinion report has been created or services have been terminated for any reason.  The website: www.Elite2ndREAD and the company: EliteRad Radiology Services accepts no responsibility for any items, films or records which may be lost or stolen while being shipped to and/or from EliteRad Radiology Services.

 

Your Access to and Control Over Information 
You may opt out of any future contacts from us at any time. You can do the following at any time by contacting us via the email address or phone number given on our website: 

   • See what data we have about you, if any. 

   • Change/correct any data we have about you. 

   • Have us delete any data we have about you.

Payment for Services

You acknowledge that you, your physician, your attorney or any authorized individual acting on your behalf and who is requesting the radiology second opinion report is responsible for all fees charged by Elite2ndREAD and/or EliteRad Radiology Services for, but not limited to, fees for radiology services.  You acknowledge that you or your aurthorized agent must pay these fees in advance, and that it is your responsibility to pursue any third-party insurance reimbursment at your own expense.  You have the option to withdraw consent for this second opinion report at anytime.  But, if you withdraw your consent after payment is made, you will remain responsible for the full amount and no money will be refunded.

Restrictions on Use

No material from the Elite2ndREAD website or EliteRad Radiology Services website may be copied, reproduced, republished, or distributed in any way without express written consent of Elite2ndREAD and EliteRad Radiology Services.  

Updates

Our Privacy Policy may change from time to time and all updates will be posted on this page.
